Home Based Services
Our home based services enable us to reach out to elders who have difficulty accessing services in the community. The continuity of care is especially important for elders who require further care after being discharged from a hospital.
Our services include:
For frail and home-bound elders who are unable to utilise the services at the centres. A service that supports caregivers and less ambulant elders.
This service includes the following:
- Chronic disease management
- Medication management (e.g. prescription, medication administration and medication reconciliation)

Within the convenience of a home setting, a care staff assists the caregivers who are caring for an elder with conditions like dementia.
Provides personal care tasks such as:
- Assistance with activities of daily living (ADLs) or instrumental activities of daily living (IADLs)
- Mind stimulating activities
- Elder-sitting and respite care
- Assistance in medication
- Perform simple maintenance exercise

Home based therapy is a holistic approach towards rehab to ensure care provision in the community.
This service includes the following:
- Home physical rehabilitation
- Home based exercise training

Home Dietetics/ Tele-dietetics Services are offered to elders who require dietary advice within the convenience of a home setting.
This service includes the following:
- Provide medical nutrition therapy
- Assess nutritional well-being of elders and implement specific diet strategies for the management of chronic conditions such as diabetes and acute illnesses, or injuries
- Provide dietary counselling and customise a nutrition care plan for elders who are unable to take their meals by mouth and have to be fed through a tube
