Course Overview
This course is about equipping learners with the knowledge and skills to perform the appropriate interventions on the care plans based on client’s needs and conditions. Thus, learners will be brought through on the importance of care planning and understand the various components and settings in care planning. This will be followed by recognizing what are the precautions, contraindication and safety considerations required before proposing and implementing the care plans.
- Have the awareness and foundational knowledge and skills to care for clients with life limiting illnesses.
- Explore multiple facets of palliative care and how to integrate the patient’s family’s role in caring for the client.
- Explain the use of care plans in guiding care delivery
- Interpret precautions, contraindication and safety considerations before implementing care plans
- Assess the effectiveness of care plan interventions
- Identify appropriate changes in care plan activities to achieve desired objectives
- Implement changes in care plan activities/intervention
- Review client’s response and progress to provide feedback on care plan
