Community Based Services

Our community based services and programmes enables elders and their loved ones to be engaged in a range of group activities such as Share-A-Pot where residents can gather to prepare and consume a meal together. In addition, elders can tap on the services available within our Community Health Centre with a General Practitioners’ referral.

Our services include:

Active Ageing

Located islandwide, our Active Ageing Centres (AACs) or Active Ageing Centres (Care) (AACs (Care)) offer a range of services for active agers living within the community to lead healthier and fulfilling lives.

Some services and programmes include:

  • Active Ageing Programmes
  • Befriending and Buddying
  • Access to Care Services

Community Health Centre

Through St Luke’s ElderCare Nee Soon Central Centre, our Community Health Centre supports local General Practitioners for general health screening and conditions.

The centre provides:

  • Dietary Counselling
  • Diabetes Checks (e.g. Retinal Screening)

Wellness Kampung

Partnership between Alexandra Health System, People’s Association and St Luke’s ElderCare for a community space that serves the residents in the area.

Some of the activities include:

  • Training for Befriending
  • Morning Exercises
  • Share-A-Pot
